ENTRY REQUIREMENTS
- Applicant should have passed Uganda Certificate of Education or its equivalent
- Should have obtained minimum passes in English, Biology, Chemistry, Physics and Mathematics
- Should have passed the oral interview
- Should have applied for the course as a choice

- Print out the application form
- Fill in the required details using a blue pen plus attach passport size photo
- Scan the filled form together with the application payment slip
- Send the scanned documents to kuluvaschool@gmail.com with the Subject “Certificate Application”

DIPLOMA NURSING/MIDWIFERY EXTENSION
ENTRY REQUIREMENTS
- Applicant should posess a certificate in Nursing/Midwifery or its equivalent
- Should have practiced in the Nursing/Midwifery profession for atleast two years
- Should have have been enrolled or verified by the Uganda Nurses and Midwives Council
